I've been having this problem where other people seem to solve it (or at least I think they do) since I've been running into it over and over when projects have been going for a while. At the beginning, Cursor / Claude Code / Codex does a pretty good job of understanding the project. You keep working on it, and you make changes to auth, move files, swap out APIs, refactor things, change databases, reverse decisions, etc. A few weeks later you have the actual code saying one thing, README/docs saying something slightly different, CLAUDE.md or rules files that haven't been updated, and old chats remembering how the projects used to work.
