I work on Maetra, and this is the control model we use for consequential actions. A reviewer approves one refund envelope with a specific agent, tool, account, amount, policy version, and executor. If the execution request changes the amount or destination, authorization rejects it. If the unchanged capability was already consumed, a replay is rejected too. The 35 second video uses an illustrative refund to show the boundary.
