I'm building Origin at Avatar8 LLC. It lets you choose two languages and call a regular phone number, with speech translated in both directions and original/translated captions on your iPhone. The person receiving the call answers normally. The biggest challenge has been conversation timing: finishing a thought, translating it, and playing it quickly enough that people don't talk over the previous turn. Voice enrollment is optional and requires consent; voice likeness and translation quality can vary.
